Job Description

Make a meaningful impact by helping individuals transition from nursing facilities to community living. We're seeking a compassionate professional to provide person-centered support and guidance.

Compensation & Schedule:

  • $18-20 per hour
  • Hybrid work arrangement (5 days in office during first 90 days of training, then only 2 days required)

What You'll Do:

  • Provide interactive decision support to consumers and families in various healthcare settings
  • Develop personalized transition plans and coordinate community-based living arrangements
  • Connect with hospitals, agencies, and facilities to promote available support options

Requirements:

  • Bachelor's degree in health/social services (or equivalent experience with state authorization)
  • 2+ years of experience working with community resources and service systems
  • Strong computer skills and ability to manage databases
