Independent Living LTSS Coordinator - Join Our Team!

Are you passionate about making a difference in people's lives? We're looking for a dedicated LTSS Coordinator to help individuals navigate their healthcare journey!

What We Offer:

  • Competitive pay: $18-20/hr
  • Flexible hybrid schedule: After 90-day training period, work from office only 2 days/week

  • $250 welcome bonus after 90 days

What You'll Do:

  • Support individuals eligible for both Medicaid and Medicare
  • Coordinate and advocate for long-term support services
  • Work directly with clients to develop personalized care plans

You're a Great Fit If You Have:

  • Bachelor's degree in social work/human services OR 2 years relevant experience/3 years lived experience
  • Spanish language skills
  • Passion for working with diverse populations
  • Strong organizational and communication skills
