What We Offer:

- Flexible hybrid schedule: After 90-day training period, work from office only 2 days/week

What You'll Do:
- Support individuals eligible for both Medicaid and Medicare
- Coordinate and advocate for long-term support services
- Work directly with clients to develop personalized care plans
You're a Great Fit If You Have:
- Bachelor's degree in social work/human services OR 2 years relevant experience/3 years lived experience
- Spanish language skills
- Passion for working with diverse populations
- Strong organizational and communication skills
