Compensation & Schedule:
- $18-20 per hour
- Hybrid work arrangement (5 days in office during first 90 days of training, then only 2 days required)
What You'll Do:
- Provide interactive decision support to consumers and families in various healthcare settings
- Develop personalized transition plans and coordinate community-based living arrangements
- Connect with hospitals, agencies, and facilities to promote available support options
Requirements:
- Bachelor's degree in health/social services (or equivalent experience with state authorization)
- 2+ years of experience working with community resources and service systems
- Strong computer skills and ability to manage databases
