In this position, you will be assessing the care of individuals with disabilities to ensure they are getting what they need to help them live independently. This includes providing information and orientation of PCA Services through the Medicaid PCA program. You will act as the liaison to individuals with disabilities while going through the evaluation process to receive approval and services from the Medicaid PCA program. This includes identifying other community resources that may strengthen their choice for independence. You will complete an initial assessment with individuals in their home to determine their level of ability to manage their program and care. This position requires traveling to individual’s homes 2-3 days a week to assess and evaluate their care and 2-3 days in the office to process paperwork.
